Occupational Health Information

All candidates offered a position with the company must complete an Occupational Health Questionnaire, which is then submitted to our Occupational Health Services. This is essential in order to protect your health and that of the company’s service users. All information and documentation provided will remain confidential and stored in compliance with the Data Protection Act.

Candidates must supply original written evidence of the following vaccinations:

Varicella (CHICKEN POX) You must confirm you have had chicken pox or shingles or a blood test result showing varicella immunity.
Tuberculosis We require an occupational health/GP certificate of a positive scar or a record of a positive skin test result.

Rubella, Measles & Mumps Certificate of “two” MMR vaccinations or proof of a positive antibody for Rubella & Measles.
Hepatitis B You must provide a copy of the most recent pathology report showing titre levels of 100lu/l or above if possible or antigen status if titre level is below 100lu/l. The report must be an identified validated sample. (Report must be IVS)
For candidates wishing to work in Theatres additional clearance information may be requires Validated proof of Hepatitis B Antibodies below 100Miu/ML (Lab serology report or a letter from your GP or OH). One negative Heb B surface Antigen or Hepatitis anti-core result following completion of the Hepatitis B course. Hepatitis C Antibody (Hep C Ab) test.

Nursing Solutions Limited will need to see original documentation and will retain photocopies. This documentation will be kept within the candidates Personnel File.

Candidates will also need to send a copy of their original documentation along with their completed Occupational Health Questionnaire to the company’s Occupational Health Services.

Offers of work are subject to a positive ‘Fitness to Work Certificates’ issued by the company’s Occupational Health Services.

If you do not have the vaccinations mentioned you will need to contact your GP or your nearest NHS Plus site to arrange immunisation.

If you do not have original copies of the required documentation please contact your GP or your Occupational Health Department at your work, and request a printout of your vaccination record.
